OUR HOME HEALTH TEAM

Skilled Nursing
Our nurses are the core of our home health medical team. In most cases, one of our nurses will be assigned as “case manager” for your care. Our nurses evaluate your condition on each visit and work with your physician to develop a goal-driven treatment plan.
Physical Therapy
Our physical therapists will help you return to your best possible level of function after an illness or injury. Their goal is to empower our patients to maintain their independence by keeping them active.
Occupational Therapy
Occupational therapists help you regain the ability to perform activities of daily living like eating, caring for your yourself, grooming. He or she will design a program of exercises and may provide assistive devices to allow you to achieve highest possible level of independence.
Speech Therapy
Speech and language therapy provides treatment, support and care for patients who have difficulties with communication, or with eating, drinking and swallowing.
Home Health Aide
Our Home Health Aides will help you with your daily activities until you are ready to be independent. Our Aides help with therapy-driven exercise, and personal care and grooming needs.

MEDICARE QUALIFYING CRITERIA

  • Patient is under the care of a physician
  • Patient requires skilled care
  • Patient is homebound and cannot leave home
  • Skilled services must be provided on an intermittent basis

If you feel you meet these criteria, talk with your physician about receiving home care services. We can also arrange private care, equipment or respiratory therapy.
